A STUDY ON THE SYSTEMIC BIOLOGIC CONTRIBUTING FACTORS OF MYOGENOUS PAIN PATIENTS IN HEAD AND NECK REGIONS

Abstract
It is essential to evaluate various contributing factors for the assessment of temporomandibular disorders which is the most common diagnosis of nondental pain in the maxillofacial area. Some studies focused on the oral parafunctional habits and emotional factors that can complicate the treatment of problems. However, the reports on systemic biologic contributing factors, known as perpetuating factors in the muscular pain, are sparse.
This study was performed to evaluate systemic biologic contributing factors in the 50 temporomandibular disorders patients with various degree of myogenous pain. Complete blood count, erythrocyte sedimentation rate, calcium, potassium, glucose, muscle enzymes(creatine kinase, lactic dehydrogenase), folate, vitamin 812, and thyroid tests were performed.
The authors came to the following conclusions :
1. Most patients (n=43, 86.0%) showed both arthrogenous and myogenous pain, and some of them(n=7, 14.0%) showed only myogenous pain.
2. Means and standard deviations of craniomandibular index(CMI), dysfunction index(Dl), and palpation index(PI) were 0.23¡¾0.11, 0.24¡¾0.13, and 0.22¡¾0.17, respectively.
3. Seventeen(34.0%) patients were anemic condition, lower than 13g/dL of hemoglobin(l2g/dL in females) or 39% of hematocrit(36% in females).
4. Fifteen (36.6%) patients showed folate deficiency and one(2.4%) showed vitamin B_12 deficiency.
5. Decreased calcium level was found in three(6.5%) and decreased potassium level was
not found.
6. The increases of muscle enzymes such as creatine kinase(CK) and lactic dehydrogenase(LDH) were found in four(l0.0%) and one(2.5%), respectively.
7. Decreased blood glucose level was not found.
8. No patients showed the laboratory findings suggesting hypothyroidism in thyroid tests.
According to the above results, it is necessary to check anemic condition and vitamin levels for the evaluation of temporomandibular disorders patients with myogenous pain.
